SCHEDULING AND NEW CLIENT PAPERWORK

We are open Monday through Friday, 9:00 am - 7:00 pm by appointment. Our office is open select Saturdays. Hours are subject to change. Avenues for Change providers may be available to schedule clients outside of regular office hours.

Already scheduled? Existing clients can connect to our TherapyPortal system to complete new client paperwork, confirm, cancel, or change appointments and to make payments. If the portal is deactivated or hasn't been set up, please notify the office or the client's clinician.
